Tell us who needs the COI

Include the certificate holder's full name and address — exactly as they've requested it. If they sent you a template or sample, attach it.

Specify any additional insureds

Let us know if the requesting party needs to be added as an additional insured. We'll handle the endorsement if required.

We issue the certificate directly from the carrier and send it to you and the certificate holder. If the requesting party has special requirements, we flag them for you.
